Plant-based and vegan diets
Strong observational evidence for the pattern, weaker evidence for the label, and a small number of nutrients that genuinely require attention rather than optimism.
"Plant-based" covers a wide range, and the range matters more than the word. It runs from vegan - no animal products at all - through vegetarian, to a diet that is mostly plants with some fish or meat. Each has a different evidence base, and lumping them together is how both advocates and critics end up talking past each other.
What the evidence shows
Large cohort studies consistently associate diets high in plants with lower rates of cardiovascular disease, type 2 diabetes and all-cause mortality. That association is robust across independent populations, which is meaningful. It is also observational, and people who eat this way in the UK tend to smoke less, drink less, exercise more and be wealthier - all of which independently affect the same outcomes. Good studies adjust for these things; adjustment is never complete.
Where randomised evidence exists it is mostly on intermediate markers rather than hard endpoints. Replacing saturated fat from animal sources with unsaturated fat from plants lowers LDL cholesterol reliably in trials, which is a real mechanism rather than an association. Fibre intake rises, with measurable effects on the gut microbiome and on satiety.
The distinction that keeps showing up in the better analyses is quality. A diet built on legumes, whole grains, nuts and vegetables looks very different in the data from one built on refined carbohydrate and processed meat substitutes - even though both are technically plant-based. Some analyses that separate "healthful" from "unhealthful" plant-based patterns find the benefits attach to the former and largely vanish for the latter. The label alone does not carry the effect.
The nutrients that actually need attention
This is the part where honest advice differs from advocacy. A well-planned vegan diet is adequate, and the UK dietetic bodies say so - but "well-planned" is doing real work in that sentence, and a small number of nutrients need deliberate management rather than assumption.
- Vitamin B12 is not reliably available from plants. Supplementation or fortified foods is not optional on a vegan diet, and deficiency causes nerve damage that can be permanent if it goes on long enough.
- Iron from plants is absorbed less efficiently than iron from meat. Intake usually needs to be higher, and vitamin C alongside it helps absorption.
- Long-chain omega-3s (EPA and DHA) come mainly from fish. The plant form, ALA, converts to them poorly in humans - algae-derived supplements are the direct route.
- Vitamin D matters for everyone in the UK, plant-based or not, given the latitude and the winter.
- Iodine, zinc, calcium and adequate protein need thought rather than panic - all are achievable, none is automatic.
Where the evidence stops
The healthy-user problem is the big one: almost all the long-term evidence is observational, and the people it observes differ from the general population in ways that also improve health. There is no large randomised trial of veganism with hard endpoints, and there probably never will be, because you cannot blind or enforce a diet for decades. Much of the confident argument on both sides is built on that same observational base, read in opposite directions.
